Most of us know what matters to us. Where we would want to be cared for, who we would want nearby, what we would want a funeral to feel like. Very few of us write any of it down, and when death comes the people we love are left guessing at every turn.
We sit down together and work through it gently, at your pace. That can mean an advance care plan, funeral wishes, letters to family, or simply getting your paperwork into one place so nobody has to search for it. Nothing is rushed and nothing is signed until you are sure.
You can do this in one sitting or over several visits, at home or somewhere you feel comfortable.

Questions people ask
Is this only for people who are dying?
No. Many people plan years ahead, in good health, because they want it done. Others come after a diagnosis. Both are welcome and neither is too early or too late.
Do I need to involve my family?
Only if you want to. Some people bring family into the conversations, others prefer to hand them a finished plan. We go at whatever pace feels right for you.
Is an advance care plan legally binding?
Some parts can be, such as an advance decision to refuse treatment, and we will be clear with you about which parts those are and how to make them count.
